Harlem Pop Warner Registration

 

Please review the below and ensure ALL steps are completed to register! Any missing steps will result in an incomplete registration.

• Complete Online Registration - LINK TO ONLINE REG

• Review and Sign Parent Contract - LINK TO GOOGLE FORM PARENT CONTRACT

• Review and Sign Code of Conduct - LINK TO GOOGLE FORM CODE OF CONDUCT

• Submit Birth Certificate via email to compliance.hpw@gmail.com (Please note, if your athlete participated last year, you are not required to complete this step again.)

• Submit Proof of Residency (Must be submitted using google form, please see form for current acceptable proofs) - LINK TO PROOF OF RES FORM

• Email 2026-2027 report card with ALL FOUR TERMS to harlemscholastics@gmail.com, this will be received after the school year and should be turned in no later than XXXX.
